When you are ready to make an appointment, contact use and you will receive an email that will link you into our client portal to complete enrollment forms. These forms are generally pretty standard and will include things like a Consent To Treat; Confirmation of Payment Policies; HIPAA Confidentiality Acknowledgement; and so on. Here are examples of how these forms may appear (please note: the specific forms you receive may be slightly different or updated -- these are for reference only).
